A separate application should be completed for each related company so they will each be listed on the Employer Registry. Alternatively, the QCARE Designation will be considered to extend to each related company listed on the applicant’s Form DWC-5 filed with the Texas Division of Worker’s Compensation and covered by the same qualifying injury benefit program.
